At Wright Mind Family Psychiatry, we are revolutionizing mental health treatment with compassion and innovation allowing each person to feel safe and supported! We have three Psychotherapists in the office, two of which are currently accepting New Patients. To request an appointment, kindly fill out the New Patient Intake form, and we will reach out to you to schedule your initial appointment. PLEASE NOTE: It is the patient’s responsibility to complete ALL intake forms along with uploading your current insurance and identification card (front and back). Should you NOT complete all required intake forms or correctly upload your insurance card (front and back) into your portal, we will have no other option than to cancel your appointment and have you rescheduled AFTER all the correct documents are uploaded securely to your patient portal. PLEASE NOTE: Please be aware that our policy requires clients to pay for their FIRST appointment out of pocket at the time of service. While we confirm the eligibility details of your insurance, we are more than happy to submit claims for subsequent appointments. We also encourage you to view your insurance benefits before your scheduled appointment, as your co-pay AND/OR coinsurance is paid in full at the time of service.
