We have one therapist in the office who is currently accepting New Patients. I'm currently focusing on psychiatric medication prescribing, and Alexis will be prescribing for our clients here by the first of the year. To request an appointment, kindly fill out the New Patient Intake form, and we will reach out to you to schedule your initial appointment. PLEASE NOTE: It is the patient’s responsibility to complete ALL intake forms along with uploading your current insurance and identification card (front and back). Should you NOT complete all required intake forms or correctly upload your insurance card (front and back) into your portal, we will have no other option than to cancel your appointment and have you rescheduled AFTER all the correct documents are uploaded securely to your patient portal. PLEASE NOTE: Please be aware that our policy requires clients to pay their copay at the time of service. If you require paperwork for leave from work, etc., we are able to provide that paid service as well.
